For example:

Makes home visits to the sick.
Ensures continuity of care following hospital discharge.
Teaches patient care to the primary care giver.
Refers those in need to appropriate agencies.
Conducts an immunization clinic for residents.
Provides free TB tests.
Conducts blood pressure monitoring.
Furnishes educational materials and teaches health classes.
Conducts bloodborne pathogen classes for City of Springdale employees.
Collaborates with schools and community agencies to promote public health.
Conducts a Lipid/Cholesterol/Glucose testing monthly for Springdale residents.
Partners with the Sharonville Health & Recreation Departments to present an annual health fair for Princeton Schools' third graders.
Investigates communicable disease cases in the community and follows up with appropriate actions.
